Answermeform.com has reopened!
at AMF (answermeform.com) you can create contact forms which you'll then be able to receive responses to them.
Currently, each user will only be able to create 2 contact forms. however this will be lifted in time, as well as the ability to purchase more contact forms and response's credit.
